Optimus Health Care is a Federally Qualified Health Care Center with sites in Bridgeport, Stamford, and Stratford, CT. We are the largest provider or primary health care services in Fairfield County, proudly serving all patients regardless of their income, race, or insurance status.
The Screening Brief Intervention and Referral to Treatment (SBIRT) Health Educator will screen and engage adults at Optimus Health Care to assess for at-risk substance use. The Health Educator will provide appropriate brief interventions or referrals to treatment by linking eligible individuals to service provision and performance monitoring programs. The Health Educator, through rigorous training and close, ongoing quality assurance monitoring, will receive about 75 hours of intensive instruction in SBIRT service protocols, during which the Health Educators will learn to provide high-quality services to adults. The Health Educator will be racially and ethnically matched, to the extent possible, to the targeted community, including having the ability to speak Spanish. This is grant funded and there is not a guarantee of this position long-term. Continuation of employment for this position is contingent upon grant funding.

Qualifications:
Must possess valid driver's license and provide own transportation for in-state travel.
Languages: Must be Bilingual (English & Spanish).
Associates' Degree and/or experience in a community health, substance abuse, social work, or clinical setting.
Sensitivity to healthcare needs of underserved and vulnerable populations.
Computer skills, including word processing, database and spreadsheet management, email and Internet.
Strong interpersonal skills, oral and written communication skills.
Excellent organizational skills.
Additional General Requirements:
Professional, positive attitude, vision and understanding of customer service principles.
Familiar with the services offered at Optimus, familiar with the neighborhoods, services and community at large.
Sensitivity to the healthcare needs of underserved and vulnerable populations.
